Show only my current treatment records (Default)
A list of those clients who currently have an active treatment record with the user’s designated program automatically appear. An active treatment record is one for which an exit reason has not yet been entered.
Show all my treatment records
This selection results in a list of clients that have ANY treatment record with the user’s designated program. This list includes current clients along with those who have exited treatment.
If you would like to see all of the clients with treatment records with the user’s program, select the second option and then click
. If you would like to see only those clients with active treatment records, these offenders already should be listed below.
Add Treatment Record (to existing or new client)
Click the
button to add a new treatment record to a new client OR to add a new treatment record to an already existing client.

Action
Click on the
icon next to the appropriate client to view the treatment record of that client. Doing so will enable you only to view the data that has been entered already; to modify the data, you must select the edit icon.
Click on the
icon to edit the treatment record of a client.
Click the
icon to transfer a treatment record to another agency.
Click on the
icon to delete the treatment record of that client. Note that the demographic data entered on the client will not be deleted – only the treatment -specific data for the selected treatment episode will be deleted. A box will appear, asking if you are sure you want to delete this item. Select “OK” to continue and delete the information, or “Cancel” to terminate this action.
When the
and
icon are missing, this is an indication the Treatment has been ended and is no longer active.
Name
Last Name, First Name, Middle Name and Suffix from the Client section of the treatment record are displayed.
A client is displayed multiple times if the client has been in treatment multiple times and data were entered on these treatment episodes.
Birth Date
Birth Date from the Client section of the treatment record is displayed.
SSN
Social Security Number from the Client section of the treatment record is displayed (if available).
SID
State ID Number from the Client section of the treatment record is displayed (if available).
Program
The name of the program the client is using for treatment.
Information is taken from the Program field in the Program Information section of the treatment record.
Start
Start is the earliest date any treatment phase began.
Information is taken from the Start Date field in the Treatment Phases section of the treatment record.
End
End represents the latest end date entered in the treatment phase fields.
An End Date appears only if all phases that have a Start Date also have an End Date.
Information is taken from the End Date field in the Treatment Phases section of the treatment record
