Year (required)
Select years From and To
For report within same year, From and To will be the same selection
First year must be smaller than second year.
Quarter (required)
Select beginning quarter (1, 2, 3, or 4)
Select end quarters to be included
First quarter must be bigger than second quarter
Facility Type (required)
Must select at least one Facility Type
Click 
to include ALL Facility Types in report
Select a facility type and click 
to move to selected box
Hold [CTL] down to include more than one Facility Types but not all
Select 
to move all selected Facility Types (in right box) to unselected (in left box)
Select a facility type in left box and click 
to unselected (in left box)
Click 
to generate report.

Report uses Adobe pdf format
User is able to Save report to local computer
